The ACADIA Facet Replacement Clinical Study is currently being conducted at spine centers throughout the U.S. The study will compare two approaches for treating Lumbar Spinal Stenosis: |

• Surgical Decompression with Facet Replacement
(using the investigational ACADIA Facet Replacement System) |

• Surgical Decompression with Spinal Fusion
(a current standard of care for treating Lumbar Spinal Stenosis) |

Participating in the ACADIA Clinical Study |

If you qualify and choose to join the ACADIA Clinical Study, your doctor will treat your stenosis with one of two procedures: Surgical Decompression with Facet Replacement (using the ACADIA Facet Replacement System), or Surgical Decompression with Spinal Fusion (a current standard of care for treating Lumbar Spinal Stenosis). The decision as to which treatment you will receive is decided at random. As part of the study, 2 out of every 3 patients will receive the ACADIA Facet Replacement System, while 1 out of every 3 patients will receive standard spinal fusion. |

Following your treatment, you will be asked to visit your study physician’s office a total of 5 times over a 2-year period. During these visits, you will be asked to complete specific questionnaires and tests so that your doctor can measure your progress. |
